The Cost of Poor Care Transitions

Per the Centers for Medicare and Medicaid Services, the cost of poor care transitions of Medicare patients from acute care alone is $26 billion per year as of 2016. There are currently over 2,500 different quality measures that impact care transitions. The alignment of metrics is essential to meaningful assessments of gaps and opportunities for efficient care coordination. To assist organizations implementing the Transitions of Care (TOC) Standards, the ACMA TOC Executive Steering committee reviewed the existing care transitions measures and identified a larger set of 95 essential measures to align to each of the five TOC standards.
