Back injuries like the one that could have ended Steelers linebacker James Harrison's career are becoming an increasing pain for NFL players and teams. Harrison, a Pro Bowl pass rusher, had two surgeries in 2011 to repair a herniated disc in his lower back.     Most doctors headed for penalty over Medicare quality reporting

    Absent a significant change in the trajectory of Medicare's physician quality reporting system, a large majority of doctors will set themselves up for future rate cuts by failing to report enough quality measures to the federal government in 2013. A recent trends report from the Centers for Medicare & Medicaid Services shows that fewer than 200,000 physicians, out of the more than 600,000 who were eligible for the incentive program, reported PQRS measures in 2010.     States crack down on prescription drug 'doctor shopping'

    Prescription drug abuse kills more people a year in the United States than cocaine and heroin combined, according to the Centers for Disease Control. A CDC report last year said 15,000 people died as a result of overdoses of prescription painkillers in 2008 — more than three times the number in 1999. Forty-three states now have databases to keep track of who is getting prescriptions for powerful pain relievers such as oxycodone, Vicodin and Opana. More

    Investment in permanent increases to primary care fees could pay dividends in the long run, according to a recent study by the Commonwealth Fund. Overall primary care costs per beneficiary would increase by about 17 percent, but the Medicare program would reap savings on such care as inpatient and post-acute visits, procedures and home healthcare. Net Medicare costs would decline by nearly 2 percent over 10 years. More
