    Hope is Not a Strategy

    One of the most important responsibilities of any group's governance is to develop a strategic plan for the group. For some groups this is the role of the Board. In other groups, all Shareholders participate in this process.

    Unfortunately, it appears that for many groups, "Hope" is their strategy.

    I am continually surprised to find groups that have never (or rarely) conducted strategic planning retreats on an annual or semi-annual basis. This is especially surprising given:
  • Almost all of many physicians' compensation comes from the group they work in.
  • Almost all of many physicians' clinical work life takes place associated to the group they are a part of.
  • Moving ahead with significant initiatives requires the collaboration of the members of the group — there is often little that one physician can do by themselves.
  • And yet, many physicians won't allocate one day a year for a group planning session.

    Passed in 1998, the Sustainable Growth Rate (SGR) system had been "patched" 17 times, as it was an unworkable and unreasonable system. The patch maneuvering to defer SGR occurred because no one could justify cutting Medicare physician fees.

    Doc participation in meaningful use dynamic, variable

    The number of New York physicians who joined the Meaningful Use program grew significantly in its first two years, but was "dynamic" and may contribute to a "digital divide," according to a new study published in Health Affairs. The researchers, from the Weill Cornell Medical College, studied the participation rates of New York's 26,368 doctors in 2011 and 2012. Only 8.1 percent of them participated in the Medicare program in 2011 and 6.1 percent in the Medicaid program.

    Usability of certified EHR technology lacking among vendors

    When the HITECH Act was passed and meaningful use regulations were established under the Medicare and Medicaid EHR Incentive Programs, the steep rise of EHR adoption and implementation brought hope for patient care advocates. After more providers increased their use of certified EHR technology, questions still remained as to whether the quality of patient care had actually changed. Many physicians still find issues with utilizing EHR systems and the lack of EHR interoperability is causing problems for effective healthcare data exchange across multiple medical facilities.

    OIG to more closely scrutinize doctors' compensation

    An attorney with HHS' Office of Inspector General said Thursday that the OIG is hiring additional lawyers to look into taking more administrative actions against doctors. They're looking for the types of questionable payment arrangements addressed in an OIG fraud alert. The alert warned doctors entering into compensation arrangements, such as medical directorships, that their pay must be in line with fair market value for services provided.

    ICD-10 bill calls for transition grace period

    If proposed legislation is your bag, then ICD-10 truly is the gift that keeps on giving. The latest example comes in the form of H.R. 2652, also known as the Protecting Patients and Physicians Against Coding Act of 2015, introduced by Rep. Gary Palmer,R-Ala. The bill seeks to provide a grace period of two years for providers set to transition from ICD-9 to ICD-10 this October.

    The Atlantic recently published an article entitled, "The Problem With Satisfied Patients." The subtitle of the article — "A misguided attempt to improve healthcare has led some hospitals to focus on making people happy, rather than making them well" — makes the focus of the piece quite clear. This piece underscores the reality that hospital reimbursements are now being linked to patient satisfaction scores. But what would happen if we focused on nurse satisfaction?
