As an attorney and consultant to orthopaedic practices ourselves, we often play the same role and sometimes get the input of recognized orthopaedic doctors themselves as well. We find that there is little substitute when trying to get physicians and surgeons to move forward with a new concept than having them hear from colleagues who have relevant experience.

    Teaching Physician Documentation
    AAOE
    When documenting Evaluation and Management services involving residents or medical students, the teaching physician must be identified. Both residents and teaching physicians may document physician services in the medical record. The documentation must be dated, and may be dictated and transcribed, typed, hand-written, or computer-generated. Medicare does state that either the resident or the teaching physician must provide customized information, meaning the documentation must describe the specific services furnished to the specific patient on the specific date (not just boiler-plate macros.)

    Physician practices seek patient satisfaction surveys as Obamacare emerges
    Forbes
    About four in five doctor practices considered "better performers" conduct patient satisfaction surveys, which are expected to take on more importance as private insurance companies and government health plans move away from fee-for-service payment under the Affordable Care Act. The Medical Group Management Association, in a new 2013 report out this month, said "almost 80 percent" of practices the group deems better performers conducts patient satisfaction surveys. This is an increase from 76 percent of better performing practices that conducted patient satisfaction surveys in 2007.

    Deadline day: Obama administration 'on track' for website goal
    CNN
    Tech experts continued working on the problem-plagued Obamacare website, the self-imposed deadline to get the portal for enrolling in the President's signature health care reforms working properly for most users. An official with the federal Centers for Medicare and Medicaid Services said Saturday the administration was "on track to meet (its) stated goal for the site to work for the vast majority of users" — a promise Obama and other officials have been making for weeks — and another CMS official said the site is "performing well" but more updates are needed.

    CMS issues guidance on tricky meaningful use requirements

    The Centers for Medicare & Medicaid Services issued guidance to healthcare providers highlighting three requirements of Stage 2 of the meaningful use program: clinical summary, patient electronic access and summary of care. According to CMS data, about a quarter of health-care providers and hospitals who received an incentive payment through Stage 1 of the meaningful use program chose to skirt at least one of these three requirements.

    As politicians debate the prospects for successful implementation of the Affordable Care Act, or Obamacare, physicians are reportedly taking a cautious approach toward making business changes related to the law. Recent industry benchmarking data from Sageworks, a financial information company, show that by several metrics, physicians at least have a decent cushion from which to evaluate their options and react to changes.

    UnitedHealthcare Medicare Advantage cuts doctors

    UnitedHealthcare has begun telling members about the network changes. But there is now about one week before the Dec. 7 deadline for choosing new coverage next year. Timing is crucial since once they sign up, most Advantage beneficiaries are locked into their plans for the year. Losing a doctor does not constitute an exception to the rule. Insurers can drop providers any time with 30 days advance notice to members.

    Do patient portals improve healthcare?

    The impact of patient portal use on health outcomes is unclear, according to a systematic review of studies. Moreover, the researchers said, the evidence to date shows that portals are unlikely to have substantial effects on efficiency and utilization of services, at least in the short term. Physicians' use of patient portals is growing rapidly, because meaningful use stage 2 requires providers to share records electronically with patients. It's widely believed that these portals can help increase patient engagement in healthcare, so the evidence about the actual effects of patient portals is important.
