    Innovations in Clinical Practice Collaboration: Optimizing Outcomes in Bundled Payment Opportunities

    As the Patient Protection and Affordable Care Act (PPACA), generally referred to as the Affordable Care Act, becomes more widespread in its integration at the individual state level along with concomitant new payment structures, providers are faced with many new considerations and challenges in clinical care provision along with payment thereof. The current programs that exist within the Center for Innovation under the Affordable Care Act are hospital-centric Accountable Care Organizations (ACOs), medical homes, and bundled payments.

    I have accepted my nerd-like tendencies towards planning each year for the AAOE National Conference. I carefully study the schedule to ascertain where I can be at every particular moment to best benefit my practice. Missing lectures is something I am just not wired to do. Often, I debate internally between two lectures. When I am at the conference, I can sometimes be seen exiting one lecture to cover another that I hope is more germane to my needs.

    CMS Issues New Rules Meant to Curb Abuses

    The Centers for Medicare and Medicaid Services (CMS) has released a final rule today that is designed to keep providers with a history of Medicare debt or abusive billing practices from participating in the Medicare program. CMS now can deny enrollment to providers that are associated with any entity that has unpaid Medicare debt, if a practice employs someone in a managerial role who has been convicted of a felony that poses a threat to Medicare beneficiaries, and any practice or entity that engages in abusive billing practices.

    CMS extension could spell trouble for meaningful use program

    The Centers for Medicare & Medicaid Services has extended the meaningful use attestation period by another month to Dec. 31 of this year. The deadline had been Nov. 30. The extension is for eligible hospitals and critical access hospitals to attest to meaningful use for the 2014 Medicare EHR program reporting period. No reason for the extension was given, though many in the industry believe it's a sign of possible trouble for the program, and a way for the federal organization to mitigate trouble brewing for health systems.

    OA Braces: Did increased utilization contribute to billing code changes?

    A growing percentage of Medicare patients with OA related knee pain and mobility limitations are being fit with off-the-shelf corrective knee braces. Following this increase in use, CMS has imposed new billing code requirements effective October 1, 2014. Statistics from 2012 indicate Medicare provided reimbursement for over 20,500 L1843 single hinge braces routinely prescribed for unicompartment OA. An additional 19,000 L1845 braces were reimbursed by Medicare. While these dual hinge (L1845) braces are prescribed for both ligament instabilities and knee OA, a majority of the L1845 braces paid for by Medicare are prescribed for older OA patients with joint degeneration.

    As EHRs proliferate, medical students must learn to use them

    Medical students are not receiving adequate training and practice when it comes to electronic health records, states a team of educators from the Association of Departments of Family Medicine. In a letter published online in the Annals of Family Medicine, the authors argue that despite the near-total adoption of EHR technology, medical students are often forbidden from full access to the software due to the fear of the students doing something improperly, which in turn prevents them from learning how to document care the right way.

    Obama administration announces major decline in medical errors

    Infections and other medical errors that harm patients in hospitals have declined significantly, the Obama administration said, hailing the progress as a sign that new efforts to improve patients' safety are bearing fruit. From 2010 to 2013, so-called hospital-acquired conditions declined 17 percent, according to a new report from the Department of Health and Human Services. Administration officials said the declines resulted in about 50,000 fewer deaths and savings of some $12 billion.

    It's really not hard to understand why physicians are asking for a two-year delay of ICD-10 implementation. We keep telling physicians that they may have to survive without reimbursement revenue for six months after Oct. 1. Why would anyone want to go through that? But it doesn't have to go down that way.     Doctors seek another ICD-10 delay

    The twice-delayed ICD-10 implementation was supposed to have taken effect on Oct. 1, 2014. Now physicians groups in two states are asking members to implore their representatives in Congress for a two-year postponement. Physician associations in Texas and New York are urging their members to press the lame duck Congress to delay ICD-10 implementation until October 2017.

    ONC lays groundwork for 'EHR of the future'

    Building on the Robert Wood Johnson Foundation's idea to incorporate data from beyond healthcare, the ONC is hoping to lay the groundwork for an "EHR of the future" that includes a wide swath of data that it said could more accurately capture a person's overall health.
