ICIM is a medical association of physicians dedicated to exploring and teaching integrative treatments to help our patients. Sept. 24-28 we gather in Dearborn, Michigan, for our conference "End Pain." This newsletter is designed to prepare attendees and continue discussions of the education we provide.


Treatment of drug and alcohol dependency and chronic pain with intravenous amino acids

It is well-known that mind- and mood-altering substances, which include prescription drugs as well as alcohol, tobacco and illegal drugs, can cause dependence and tolerance through desensitization of the neurotransmitter receptor or similar damage to the synapse. This physiologic alteration makes cessation of substance use difficult, it accounts for many of the side effects that accompany drug use, and creates the situation that even after reduction or cessation of use the brain may not be able to repair and restore normal neurotransmission. Hence a lasting degree of anxiety, pain, depression, mental fog, cravings and/or insomnia are typical during or after drug dependence.

The quickest way for an athlete to lose strength at the ligament-bone junction (fibro-osseous junction) is to inject cortisone to that area. Cortisone and other steroid shots ALL have the same detrimental effects on articular cartilage. Corticosteroids, such as cortisone and Prednisone, have an adverse effect on bone and soft tissue healing. Corticosteroids inactivate vitamin D, limiting calcium absorption by the gastrointestinal tract, and increasing the urinary excretion of calcium. Bone also shows a decrease in calcium uptake with cortisone use, ultimately leading to weakness at the fibro-osseous junction. Corticosteroids also inhibit the release of Growth Hormone, which further decreases soft tissue and bone repair. Ultimately, corticosteroids lead to a decrease in bone, ligament and tendon strength.

EDTA redistribution of lead and cadmium into the soft tissues in a human with a high lead burden — should DMSA always be used to follow EDTA in such cases?

Intravenous sodium calcium ethylene diamine tetra acetic acid and oral 2,3-dimercaptosuccinic acid have both been used to reduce the burden of lead in humans. Each of these agents enhances the mobilization of lead from different areas of the body — EDTA from the trabecular bone and DMSA from the soft tissue. A study of Korean battery workers revealed that EDTA appeared to increase the soft tissue burden of lead, resulting in increased levels of aminolevulinic acid and greater subsequent lead mobilization with DMSA. This case report discusses a patient with a higher-than-normal lead burden who exhibited increased tissue lead burden after intravenous EDTA.
