Integrative medicine: Bridging the gap between 'traditional' and 'alternative' medicine
Is it semantics or is there really a difference between so-called traditional and so-called alternative medicine? I have come to wonder which is which? In my practice I have gone from being "open" to herbs and supplements patients are taking, to actually understanding the reason why patients seek out alternative methods to the present time - where I not only support alternative methods but actually prescribe them. I am finally at a point in my career where I humbly realize that Western Medicine only has a place in the diagnosis and treatment of our patients, and that we, as practitioners, are here to guide and direct patients towards the treatments that are best for them, personally. I don't call this Integrative Medicine...I call it Personalized Medicine, as it should be. Indeed, it is all about the language in which we share ideas between East and is this language that will bridge the gap, and enable to help patients...what we went into this business to do in the first place.

Clinical discussion on oral contraceptives and testosterone

Q: I am writing a new book. The topic is how to prevent the side effects from birth control pills. My question to any of you have experience with:
a) putting women who are taking birth control pills on low dose transdermal testosterone supplementation....and is it effective at restoring libido?
b) putting women taking Oral Contraceptives on natural progesterone to help prevent or correct side effects from OCs?

A: I would prefer to get them off oral contraceptives all together for the reasons you list. Many women have needed bio-identical progesterone and at times testosterone for a time, until their own pituitary reregulates hormone production. I recommend doing an expanded salivary female hormone panel throughout one full cycle (I use Diagnostechs) but wait 2-3 months after discontinuing the synthetic hormones. This will assist you in assessing how to manage the patient (although their recommended dosages tend to be on the lower side and some women may need more.) Progesterone is essential to re-regulate and restore ovulation. Several years ago, I was able to use progesterone to restore a normal ovulatory pattern in a 45 year old woman who had been on OC's for many, many years, then multiple courses of IVF that failed. Result: she had a normal term pregnancy and delivered a beautiful baby girl at age 47.

However, where discontinuing is not an option, I have added natural progesterone 25 to 100 mg range twice a day (caps in oil or troche as cream is harder to regulate the cycle, in my experience) during the second half of the cycle. My experience with doing this has been very positive with most patients responding well. Physiologic doses of testosterone for women are in the range of 2-10 mg but please follow blood or saliva to avoid over-dosage. Quest has lowered its reference range considerably (based on post-menopausal values) but an optimal level in serum is 25-75. You can use a vaginal preparation directly on the labia which helps sexual function and libido.

In either case, be sure to replete nutrients (especially zinc, B vitamins including Biotin, etc.).

Health freedom-loving New Jerseyans are concerned by state senator Loretta Weinberg's Senate Bill No. 1759. Introduced in March, Weinberg's bill is a bid to make the exercise of a vaccine religious exemption more difficult for New Jersey parents. If passed, this bill would require parents to state their religious beliefs to exempt their children from vaccines for school enrollment, something the current N.J. exemption law doesn't require. The New Jersey bill would add New Jersey to a growing list of states that have recently passed laws restricting access to exemptions. Last year, Washington state — and this year, California — passed laws making medical doctors gatekeepers of nonmedical exemptions. These laws don't pass the common sense test, let alone formal legal scrutiny. States cannot lawfully put a doctor between a citizen and the exercise of their religious beliefs, which has nothing to do with medicine. More
