More Proof of Declining Paravalvular Regurgitation With Newest Balloon-expandable TAVR Devices
Two recent, single-center studies provide reassurances that both the incidence and severity of paravalvular regurgitation are reduced among TAVR patients treated with the newest iteration of a balloon-expandable valve as compared with earlier devices. Moreover, data suggest that dwindling rates of paravalvular leak are related to device refinement, rather than differences in aortic stenosis severity or operator technique, one of the studies adds.

In U.S. cath labs, fractional flow reserve (FFR) utilization is growing. Whereas overall use was reported at 6.1 percent in percutaneous coronary intervention for intermediate coronary stenosis in 2012 (J Am Coll Cardiol 2012;60:2337-9), more contemporary FFR utilization is estimated to be 20-25 percent in the intermediate lesion population. The SCAI Quality Improvement Toolkit encourages use of FFR in intermediate lesions as a quality initiative that should be tracked.

The U.S. Department of Health and Human Services (HHS) Office of the National Coordinator for Health Information Technology (ONC) recently released a request of information (RFI) on how to measure interoperability across health care environments. The Medicare Access and CHIP Reauthorization Act (MACRA) directs HHS to work with the health care community and technology developers to establish metrics to determine if, and to what extent, interoperability exists nationwide.
SCAI signed on with multiple medical associations in regard to overarching concerns with the actions that both the ONC and the Centers for Medicare and Medicaid Services are taking to improve interoperability. SCAI and the signing organizations are concerned that without significant changes to the health information technology measures themselves, electronic health record vendors will continue to develop products that meet federal reporting requirements rather than the needs of patients and physicians.

In a study published online by JAMA Cardiology, Elizabeth Selvin, PhD, MPH, of the Johns Hopkins Bloomberg School of Public Health, Baltimore, and colleagues examined the association of six-year change in high-sensitivity cardiac troponin T with incident coronary heart disease, heart failure and all-cause mortality. High-sensitivity cardiac troponin T, a protein that can be measured via a blood test, is a biomarker of cardiovascular risk and could be approved in the United States for clinical use soon.
