Report faults training, communication errors for White House breach

from CBS News

Due to a host of "performance, organizational, and technical" failures, an intruder was able to scale the White House fence in September and make his way inside the executive mansion before he was taken down by Secret Service agents, according to a report on the incident by the Department of Homeland Security that was made public.
