One evening, my 92-year-old patient was not as responsive as usual and not easily aroused. His vital signs were: blood pressure, 107/67 mm Hg; pulse, 67 beats/min; respiratory rate, 45 breaths/min; temperature, 98.1° F; and oxygen saturation by pulse oximetry, 66 percent. I called my supervisor immediately, and we both agreed that the patient should be transferred to the acute care hospital. I called the attending physician; and, after explaining the situation with the use of SBAR (situation, background, assessment, recommendation), he agreed to transfer the patient to the emergency department. more
