Can we just cut the medical record bloat?

from MedPage Today (opinion)

Fred N. Pelzman, MD, writes, "With our modern electronic medical records, which sometimes function as fancy word processors, it's easy to insert templated material using macros that spit out huge chunks of text — the things we type over and over — and it's supposed to make our lives easier. But often I feel that if I'm punching that macro button — those few letter shortcuts that blossom onto the page into a massive satisfaction of documentation — then maybe it really wasn't worth typing at all. 'Patient advised to return to the Emergency Department for fevers over 101, chest pain, bleeding, abdominal pain, shortness of breath, headache, confusion.'" more
