The UCLA Immunogenetics Center (UIC) provides comprehensive testing for organ and tissue transplantation. Transplant testing has a long history at UCLA. HLA typing was pioneered here in the 1960's. The development of the microcytoxicity test in 1964 marked the beginning of international testing and standardization of HLA typing. The UCLA Immunogenetics Center has retained its leadership position in HLA research, and in the development of accompanying diagnostic testing. MORE

There are APOL1 gene variants, principally within the black population, that confer additional risk of chronic kidney disease. This raises the ethical question of whether to test prospective donors for these variants, and whether the presence of such a deleterious genotype should preclude one from donating. In the latest AST blog, Past-President Ken Newell discusses a fantastic conference that the AST hosted on the biology and ethics surrounding APOL1 gene variants and organ donation.

A report from a high-volume single center indicated a survival benefit of receiving a kidney transplant from an HLA-incompatible live donor as compared with remaining on the waiting list, whether or not a kidney from a deceased donor was received. The generalizability of that finding is unclear.

Although antibodies against the programmed death 1 (PD-1) receptor and one of its ligands (PD-L1) have produced tumor regressions in multiple cancer types, these therapies are untested in patients treated with long-term immunosuppressive medications. Here, we report a case in which PD-1 antibody was administered to a solid-organ transplant recipient with metastatic cutaneous squamous-cell carcinoma.

The health benefits of PA are well established for healthy and chronically ill children. This study objectively measures physical fitness and PA levels in children PLT and explores potential correlates and perceived barriers impacting their PA. A total of 23 children (16 females, mean 14.01 plus or minus 2.49 years) greater than 1 year PLT were assessed for peak oxygen consumption (VO2peak), muscle strength, endurance, and flexibility.

The number of donor organs suitable for liver transplantation is restricted by cold preservation and ischemia–reperfusion injury. We present the first patients transplanted using a normothermic machine perfusion device that transports and stores an organ in a fully functioning state at 37°C. In this Phase 1 trial, organs were retrieved using standard techniques, attached to the perfusion device at the donor hospital, and transported to the implanting center in a functioning state.

The National Organ Transplant Act stipulates that deceased donor organs should be justly and wisely allocated based on sound medical criteria. Allocation schemes are consistent across the country, and specific policies are publicly vetted. Patient selection criteria are largely in the hands of individual organ transplant programs, and consistent standards are less evident. This has been particularly apparent for patients with developmental disabilities (DDs).

As medical advances are made in the care of persons with chronic illnesses including those with end-stage renal disease, patients are not only experiencing increasing life expectancy but also bearing the burden of illness and treatment for a longer duration of time. With this in mind, it is increasingly important for health care providers to pay close attention to their individual patient's perceptions of their health, fitness, life satisfaction, and well-being.
