The Centers for Medicare & Medicaid Services (CMS) published the original Interpretive
Guidelines in 2008 in policy memorandum SC-08-25, which were later updated through Survey
and Certification (S&C) policy memoranda. CMS has now updated the Organ Transplant
Interpretive Guidelines to incorporate previously-published changes, clarify certain areas, and
address feedback received based on previously-released drafts.

The UCLA Immunogenetics Center (UIC) provides comprehensive testing for organ and tissue transplantation. Transplant testing has a long history at UCLA. HLA typing was pioneered here in the 1960's. The development of the microcytoxicity test in 1964 marked the beginning of international testing and standardization of HLA typing. The UCLA Immunogenetics Center has retained its leadership position in HLA research, and in the development of accompanying diagnostic testing. MORE

As part of an effort to engage the medical community in enhancement of the Maintenance of Certification (MOC) program, the American Board of Internal Medicine (ABIM) is reaching out to all physicians board certified in nephrology for feedback on the MOC exam blueprint. The blueprint is the tool ABIM uses to design an exam to assure content appropriately covers the field.

There are APOL1 gene variants, principally within the black population, that confer additional risk of chronic kidney disease. This raises the ethical question of whether to test prospective donors for these variants, and whether the presence of such a deleterious genotype should preclude one from donating. The introduction of the Mayo End-Stage Liver Disease score into the Organ Procurement and Transplantation Network (OPTN) deceased donor liver allocation policy in 2002 has led to a significant increase in the number of simultaneous liver–kidney transplants in the United States. Despite multiple attempts, clinical science has not been able to reliably predict which liver candidates with renal insufficiency will recover renal function or need a concurrent kidney transplant.

Sarcopenia is the most common complication of cirrhosis and adversely affects quality of life and outcomes before, during, and after liver transplantation. We studied predictors of sarcopenia and sarcopenic obesity in patients with cirrhosis undergoing liver transplant (LT) evaluation. A retrospective analysis of 207 adult cirrhotic patients that underwent LT from January 2008 to December 2013 was performed at our institution.

Clinical Transplantation
The use of public insurance is associated with diminished survival in patients with cystic fibrosis (CF) following lung transplantation. No data exist on benefits of gaining private health insurance for post-transplant care among such patients previously using public insurance. The United Network for Organ Sharing database was used to identify first-time lung transplant recipients participating in Medicare or Medicaid, diagnosed with CF, and transplanted between 2005 and 2015. Survival outcomes were compared between recipients gaining private insurance after transplantation and those maintaining public coverage throughout follow-up.

Heterologous immunity is recognized as a significant barrier to transplant tolerance. Whereas it has been established that pathogen-elicited memory T cells can have high or low affinity for cross-reactive allogeneic peptide–MHC, the role of TCR affinity during heterologous immunity has not been explored. We established a model with which to investigate the impact of TCR-priming affinity on memory T cell populations following a graft re-challenge.
