In partnership with the AST, the ASTS, and the Arnold Foundation, we are conducting a survey about transplant quality/regulatory metrics. Please complete our brief survey by April 15, 2017 to help ensure that most appropriate metrics are used to regulate transplant centers. Responses are anonymous. Results will help us better understand the strengths and weaknesses of measures currently used for CMS/SRTR quality regulation and inform the design of alternative metrics.
Take the survey.

Later this year, nine states will require surgeons to report information on the number of postoperative visits they provide their patients. In this issue of “The AJT Report,” we take a look at how this change in reporting may impact transplantation. Also this issue, we review the results of liver transplantation in recipients with the hepatitis C virus.

The prevention, diagnosis, and management of infectious disease in transplantation are major contributors to improved outcomes in organ transplantation. The risk of serious infections in organ recipients is determined by interactions between the patient's epidemiological exposures and net state of immune suppression. In organ recipients, there is a significant incidence of drug toxicity and a propensity for drug interactions with immunosuppressive agents used to maintain graft function.

Thirteen percent of individuals of African ancestry express two variant copies of the gene encoding apolipoprotein 1 (APOL1) that has been associated with an increased risk of end-stage renal disease (ESRD) in the general population. Limited studies suggest that the survival of transplanted kidneys from donors expressing two APOL1 risk alleles is inferior to that of kidneys from donors with zero or one risk allele.

Frailty is associated with inferior survival and increased resource requirements among kidney transplant candidates, but assessments are time-intensive and costly and require direct patient interaction. Waitlist hospitalization may be a proxy for patient fitness and could help those at risk of poor outcomes. We examined United States Renal Data System data from 51 111 adult end-stage renal disease patients with continuous Medicare coverage who were waitlisted for transplant from January 2000 to December 2011.
