The AST's Transplantation and Immunology Research Network (TIRN) has issued two RFPs in collaboration with outside organizations; proposals are due May 1. Investigators interested in improving transplant center metrics can apply for funding from the Laura and John Arnold Foundation. Researchers with projects in 3-D bioprinting or bioreactor development can compete for a $50,000 grant from the Human Organ Project.

The UCLA Immunogenetics Center (UIC) provides comprehensive testing for organ and tissue transplantation. Transplant testing has a long history at UCLA. HLA typing was pioneered here in the 1960's. The development of the microcytoxicity test in 1964 marked the beginning of international testing and standardization of HLA typing. The UCLA Immunogenetics Center has retained its leadership position in HLA research, and in the development of accompanying diagnostic testing. MORE

The aim of this prospective, longitudinal study was to determine the role of these pathophysiological variables in acute–on–chronic liver failure (ACLF) patients with and without hepatic encephalopathy (HE). This study describes potential mechanisms of HE in ACLF indicating that ammonia and abnormal cerebral oxygenation are important and the results suggest that ammonia, JVO2 and WCC are important prognostic biomarkers and therapeutic targets

While immune checkpoint therapy has improved outcomes for many patients with cancer, it could activate a robust immune system that attacks and rejects a transplanted organ. This month, "The AJT Report" explores what this new therapy means for the care of transplant patients who develop cancer. Also this month, we look at updates to candidate criteria for heart transplantation, and at new data from UNOS indicating that 2015 was a record-breaking year for the number of transplants performed in the U.S.

Solid organ transplantation has been an area of great interest to commercial payers ever since it moved into mainstream medical care beginning in the 1980s. Over the years a system of evaluating transplant program performance based on volume and one year graft and patient survival has developed. While this system has served its purpose, there is an increasing realization from payers that a need exists for a more sophisticated way to evaluate quality and cost-effectiveness of these complex procedures. We report on the perspective of a large transplant network and its efforts to better understand the drivers of value over the entire continuum of care from referral through one year post-transplant.

The most difficult, and perhaps, most important decision that a clinician makes for a patient on the liver transplant wait-list is when 'not' to proceed with liver transplant. Although an individual may be suitable for transplant surgery at listing, he/she may become too sick while waiting. This article reviews four specific conditions that commonly arise on the wait-list that may render a candidate too sick for transplant: advancing age, sarcopenia, acute on chronic liver failure, and nonliver-related medical comorbidities.
