For donation after circulatory death (DCD), many centers allow 1 h after treatment withdrawal to donor death for kidneys. Our center has consistently allowed 2 h. We hypothesized that waiting longer would be associated with worse outcome. A single-center, retrospective analysis of DCD kidneys transplanted between 2008 and 2013 as well as a nationwide survey of organ procurement organization DCD practices were conducted.

Solid organ transplant recipients (SOTRs) are at increased risk of developing and dying from cancer. However, controversies exist around cancer screening in this population owing to reduced life expectancy and competing causes of death. This systematic review assesses the availability, quality and consistency of cancer screening recommendations in clinical practice guidelines (CPGs).

Despite generally positive outcomes and high rates of satisfaction, living kidney donors are at risk for both medical and psychosocial problems. This review summarizes non-ESRD risks for donors, and describes limitations to the data. We review evidence of medical risks (e.g., increased cardiovascular disease and mortality, preeclampsia) and psychosocial risks (e.g., mood disturbance, financial burden). We discuss evidence of differential risks among subsets, and the impact of postdonation events (e.g., development of diabetes).

Pediatric Transplantation
The use of status exceptions (SE) was recently publicized as a strategy to reduce waitlist times for children awaiting heart transplant (HTx). The aim of this study was to assess SE use across UNOS regions and compare survival in patients listed using a SE to those listed by standard criteria. The OPTN database was queried for all pediatric patients listed for HTx (2000–2014). SE use was compared across UNOS regions.

Living donor solicitation can pose ethical concern, no matter the medium used: newspaper, television, pulpit, billboard, or Internet. Moving the search for a living donor into the social media realm introduces the risk of unguided and coercive patient narratives, as well as a decoupling or even total absence of information that could aid the consent process.
