The AST continues to remain focused on transplant-related proposals coming out of CMS. Leadership from the Transplant Administrators COP was actively engaged in reviewing the July 14, 2016 public comment proposal and providing feedback to AST leadership. The AST's response may be reviewed here.

AST's government relation team continues to actively monitor the ESRD Integrated Care Act (H.R. 5506 and S. 3090) due to its negative impact on kidney transplant recipients as currently drafted. A letter was submitted to sponsors of this bill in late July due to the AST's strong reservations regarding a capitated payment system that would create financial disincentive for referral of Medicare patients for transplant evaluation. Last week, the full House of Representatives Ways & Means Committee began its mark-up of the dialysis community driven ESRD capitated payment model demonstration legislation or "PATIENT Act." We will continue to monitor this important situation and keep members apprised of activity around this bill.

There are 15 proposals currently out for public comment. Please take some time to review these proposals and share this information within your institutions. Feedback is critical to the policy development process. The AST's COPs are finalizing their review of proposals this week. Their responses will then go to the AST's Policy Committee for consideration to draft a unified AST response that will then go to the Board of Directors for approval before submission by the Oct. 14 deadline.
FDA
On Sept. 27, 2016, the FDA is conducting a public meeting on Patients Who Have Received an Organ Transplant. FDA is interested in obtaining patient input on the impact of receiving an organ transplant on daily life and patients’ views on currently available therapies to manage organ transplantation. The American Transplant Congress is designed for physicians, surgeons, scientists, nurses, organ procurement personnel and pharmacists who are interested in the clinical and research aspects of solid organ and tissue transplantation. Clinical Transplantation
Previous studies have reported contradictory results regarding the effect of pre-transplant dialysis modality on the outcomes after kidney transplantation (KT). To minimize the confounding effect of donor-related variables, we performed a donor-matched retrospective comparison of 160 patients that received only one modality of pre-transplant dialysis (peritoneal dialysis [PD] and hemodialysis [HD] in 80 patients each) and that subsequently underwent KT at our center between January 1990 and December 2007.

Human Immunology
The “holy grail” of transplantation is donor specific tolerance in the absence of immunosuppression. There are no cases reported of survival without immunosuppression in heart transplant recipients. This prospective study identifies a group of heart transplant recipients who have developed tolerance to the transplanted organ. We hypothesize that these recipients were pre-disposed to survival without immunosuppression. Identification of a gene signature in these recipients could help understand the mechanism of development of transplant tolerance in heart transplant recipients.

Acute kidney injury (AKI) and chronic kidney disease (CKD) are common in patients awaiting liver transplantation, and both have a marked impact on the perioperative and long-term morbidity and mortality of liver transplant recipients. Consequently, we reviewed the epidemiology of AKI and CKD in patients with end-stage liver disease, highlighted strategies to prevent and manage AKI, evaluated the changing liver transplant waiting list's impact on kidney function, delineated important considerations in simultaneous liver–kidney transplant selection, and projected possible future transplant policy changes and outcomes.

Both acute and chronic kidney disease are common after liver transplantation and result in significant morbidity and mortality. The introduction of the Model for End-stage Liver Disease score has directly correlated with an increased prevalence of perioperative renal dysfunction and the number of simultaneous liver–kidney transplantations performed. Kidney dysfunction in this population is typically multifactorial and related to preexisting conditions, pretransplantation renal injury, perioperative events, and posttransplantation nephrotoxic immunosuppressive therapies.
