Alloimmune risk stratification in renal transplantation has lacked the necessary prognostic biomarkers to personalize recipient care or optimize clinical trials. HLA molecular mismatch improves precision compared to traditional antigen mismatch but has not been studied in detail at the individual molecule level. This study evaluated 664 renal transplant recipients and correlated HLA‐DR/DQ single molecule eplet mismatch with serologic, histologic, and clinical outcomes.

Factors that patients value when choosing a transplant center have not been well studied. The Final Rule mandates that organ allocation not be based on the transplant candidate's place of residence or listing, except as required by sound medical judgment and best use of donated organs, to avoid wasting organs and futile transplants, and to promote access and efficiency. Current Organ Procurement and Transplantation Network (OPTN) policies use donation service areas and OPTN regions to distribute and allocate organs for transplant.

Liver transplantation is a highly successful treatment, but is severely limited by the shortage in donor organs. However, many potential donor organs cannot be used; this is because sub-optimal livers do not tolerate conventional cold storage and there is no reliable way to assess organ viability preoperatively. Normothermic machine perfusion maintains the liver in a physiological state, avoids cooling and allows recovery and functional testing.

Transplantation
Liver transplantation represents an effective treatment for patients with end-stage liver disease. However, patient access to this lifesaving procedure continues to be limited by the lack of organs resulting into high mortality rates while awaiting transplantation. Attempts to expand the donor pool during the last decade using marginal organs and partial grafts have reached a plateau. At the same time, organ discard rates (organs procured but never transplanted) continue to remain mainly related to the poor quality of donor organs.
