The Trump administration wants to give the country's kidney care a facelift, as treatment in the U.S. has remained relatively unchanged since President Richard Nixon signed the Medicare kidney disease entitlement into law in 1972. But concerns over methodology and measurement could swamp one prong of the administration's plans.
Though industry agrees the country's aging kidney care system is overdue for a revamp, some stakeholders are concerned about the mandatory CMS model in Medicare that, along with four voluntary models, is meant to incentivize dialysis centers to provide high quality care in a patient's home.

Liver Allocation Study Group
An optimal allocation system for scarce resources should simultaneously ensure maximal utility, but also equity. The most frequent principles for allocation policies in liver transplantation are therefore criteria that rely on pre-transplant survival (sickest first policy), post-transplant survival (utility), or on their combination (benefit).

Head & Neck
In 2018, over 35,000 solid organ transplants were performed in the United States. Unfortunately, this population of transplant recipients have been shown to have an increased risk of subsequently developing a range of cancers. Despite the fact that solid organ transplant recipients are vulnerable to a number of medical comorbidities, cancer is the second leading cause of death for such patients, accounting for 13 percent of deaths in one recent study of 22,162 patients. The most common posttransplant malignancies include skin cancers, lymphoproliferative disorders, and lung cancer. Several studies have also demonstrated increased risk of head and neck cancers; in particular, oral cavity, oropharynx, and thyroid cancers have been shown to be associated with prior solid organ transplant.

Because cardiac and renal disease are physiologically related and often coexist, the prevalence of combined heart and kidney transplantation has significantly increased over the last few years. It has been suggested that combined organ allografts modulate the immune system favorably for one or both allografts resulting in successful clinical outcomes.

Cardiovascular events are among the leading cause of mortality in kidney and liver transplant recipients. Thus, screening for cardiovascular disease and risk stratification for cardiovascular events constitute an important part of the pretransplant evaluation.

Researchers recently developed a high throughput T cell receptor β chain sequencing-based approach to identifying and tracking donor-reactive T cells. To address the role of clonal deletion in liver allograft tolerance, they applied this method in samples from a recent randomized study, ITN030ST, in which immunosuppression withdrawal was attempted within two years of liver transplantation.
