Diabetic foot osteomyelitis remains one of the most challenging conditions to manage and treat in the foot and ankle. It is associated with high rates of recurrence, significant morbidity and mortality and costs. Diagnosis can be particularly challenging in the presence of diabetes due to an often blunted neuroinflammatory response, in which the usual manifestations of infection (e.g., fever, tachycardia, elevated white blood cell count) are frequently absent.

Mark E. Easley, MD, FAAOS, performed total ankle arthroplasty, double arthrodesis and foot realignment/stabilization in a 72-year-old man with stage 4 flatfoot deformity. Intraoperatively, varus/valgus stability is shown and there is no gapping with valgus stress. Despite minimal bone resection, the largest primary polyethylene thickness available was used to balance the patient’s ankle.
