Given that diabetes remains the most common cause of non-traumatic amputation in the U.S., these authors strongly emphasize a multidisciplinary approach and provide a compelling review of the literature to assess key factors, costs and outcomes in choosing between limb salvage and amputation in this patient population.
The definition of limb salvage is saving a lower extremity that would have otherwise been amputated. In addition to avoiding primary amputation, the goal of a limb preservation initiative is to achieve a plantigrade foot that provides the highest level of function with the shortest recovery period, even if that means a minor amputation and/or reconstructive foot surgery.

EPFL researchers have developed a shoe sole with valves that electronically control the pressure applied to the arch of the foot. The goal is to relieve foot ulcers commonly caused by diabetes and help them heal to avoid dangerous secondary infections.
Diabetes can lead to dangerous complications that all too often end in amputation of the foot or even death. To avoid these eventualities, a team of researchers from EPFL is collaborating with the Geneva University Hospitals on a smart shoe. The technology is based on actuators that electronically control the stiffness of the sole in order to reduce the onset of diabetic foot ulcers and prevent existing ones from worsening. An initial prototype has just been completed.

It's fair to characterize the relationship between physicians and Medicare as one of both
love and hate. Of course, most physicians naturally love and appreciate the opportunity to see a
multitude of patients each day, supported by this vast federal program.
On the other hand, most physicians will most likely admit to hating the increasing rigmarole involved in
getting paid for these very same patients. Podiatric physicians are, by no means, exceptions to this rule,
especially today. In recent years, for example, podiatric physicians were clobbered annually by the threat of
the sustainable growth rate, or SGR, which has since ended, and has been replaced with a new payment system. The light at the end of that long tunnel, however, has been dimmed by the onslaught of new initiatives
such as ICD-10 and mandatory PQRS reporting.

Results of a survey of 64 orthopedic surgeons presented here showed significant variability in how narcotics are prescribed for patients with foot and ankle pain. According to the researchers, these inconsistencies highlight the need for better, more clear-cut criteria on how to most efficiently use narcotics postoperatively.
Those surveyed, 39 percent of whom practiced in an academic setting, were presented with four clinical scenarios. They then answered multiple-choice questions regarding the type and dose of pain medication they would prescribe postoperatively for the different scenarios.

The perception that their physicians were empathetic was the biggest factor in patients' satisfaction, the study, which was presented at the 2016 annual meeting of the American Academy of Orthopaedic Surgeons, found. The study linked a patient's perceived physician empathy with improved outcomes and medical care satisfaction, according to an announcement.
