Mechanic's hands is a well-characterized manifestation of select idiopathic inflammatory myopathy (IIM) syndromes. Less well characterized is the hyperkeratosis of the toes and plantar surface of the feet that can also accompany these disorders. We aim to describe common pedal signs in the context of IIM, and suggest that it may be another key feature in the presentation of these syndromes. A cohort of 2145 myositis patient charts gathered since 2003 were retrospectively reviewed using the key search terms "mechanic's feet" and/or "mechanic's foot."

Severe foot pain and planus posture leave patients at a higher risk of falling, according to a recent study in Gerontology.
The study, focusing on 1,375 patients with a mean age of 69, found that 263 patients related one fall in the past year and 152 patients had two or more falls. Researchers note that foot pain was linked with a 62 percent greater risk of recurrent falls and patients experiencing moderate or severe foot pain had higher odds of two or more falls. In addition, the study notes that patients with planus foot posture have a 78 percent higher risk of falls while foot function had no association with falls.

Although accumulative evidence exists that support the applicability of multi-segmental foot models in evaluating foot motion in various pathologic conditions, little is known of the effect of aging on inter-segmental foot motion. The objective of this study was to evaluate differences in inter-segmental motion of the foot between older and younger adult healthy females during gait using a MFM with 15-marker set.

Ankle fractures are frequently occurring injuries. Despite the relatively simple operative technique, patients often suffer from postoperative complications. Little is known about postoperative treatment of implant-associated infections of the ankle. Therefore, this study shows and evaluates a treatment algorithm in long- and short-term outcomes compared to infection-free patients.

Vitamin D insufficiency and deficiency is an increasingly prevalent disorder. Numerous factors result in vitamin D deficiency, These factors include inadequate sunlight exposure, malabsorption syndromes and medications that alter vitamin D metabolism. Certain populations are at increased risk of vitamin D deficiency, including the elderly and adults in nursing homes and healthcare institutions. Diseases associated with vitamin D deficiency include celiac disease, short bowel syndrome and Crohn's disease. Medications including anticonvulsants, glucocorticoids and rifampin can increase hepatic metabolism and actively destroy vitamin D.

Management of the diabetic foot is multifaceted and requires constant monitoring from patients and health care providers. The alarmingly high rate of recurrence of ulcerations in diabetic foot requires a change in our approach to care and to the vernacular in the medical literature. With its high rates of morbidity and recurrence, care of the complex diabetic foot may be aptly comparable to many forms of cancer.

Common foot problems are independent risk factors for falls in older people. There is evidence that podiatry can prevent falls in community-dwelling populations. The feasibility of implementing a podiatry intervention and trial in the care home population is unknown. To inform a potential future definitive trial, we performed a pilot randomized controlled trial to assess: (i) the feasibility of a trial of a podiatry intervention to reduce care home falls and (ii) the potential direction and magnitude of the effect of the intervention in terms of number of falls in care home residents.

Healthcare is one of the most dynamic industries. Not only are new discoveries, modalities, equipment or medications being introduced at a rapid pace, but the unpredictability of taking care of humans also creates an extra challenge. However, one thing that remains constant is the desire of healthcare workers to provide excellent care. So the challenge is figuring out how front-line healthcare providers can implement evidence-based practice into their daily patient care.
