An estimated 40 percent of the U.S. population have foot problems. Of all patients aged over 50 years, 2.5 percent report degenerative arthritis of the first metatarsophalangeal joint, termed "hallux rigidus." First MTP osteoarthritis is the most common arthritic condition in the foot. Progression of great toe arthritis is associated with pain and loss of motion. Non-surgical intervention begins with shoe modifications and orthotics designed to limit MTP motion.

Richard Blake DPM writes: I had a patient contact me previously regarding the left foot for which he received a diagnosis of tenosynovitis and plantar fasciitis. The same patient contacted me recently regarding a separate problem in the right foot.
As for the left foot, the patient increased his activity one day at the gym using an elliptical machine that put weight on his heels and was able to work out for 45 minutes at a good intensity. The next day, the tenosynovitis was much worse and the patient realized he had been curling his toes frequently, testing them to see if they were getting better.

These authors discuss the use of tendon transfers and Achilles tendon resection for a 56-year-old patient who presented with drainage and dermatitis after receiving an allograft for an injury to the Achilles tendon.
While Achilles tendon injuries are the most common tendon injuries in the lower extremity, clinicians misdiagnose these injuries between 20 to 25 percent of the time. These injuries often occur in young athletic males due to indirect trauma or a sudden forceful contraction.
