If you have been following recent articles in Podiatry Today and PM News about the different approaches to prescribing custom foot orthoses, you may be confused and troubled. Was what you learned in your training still the best approach, or are there new theories that you are not completely familiar with? And how can you tell whether you can improve your success rate by changing approaches? ACFAOM 2016 will include a 3-hour interactive debate with nationally-known proponents of the Root, Tissue Stress, and MASS approaches. Each presenter will have time to make his case, and then panel and audience members will be able to challenge each presenter with actual cases. Podiatry Management
One of the most fundamental techniques for
the management of diabetic foot ulcers is debridement. When done properly, this process will decrease
necrotic tissue, reduce callus, eliminate biofilms, and stimulate angiogenesis, mitogenesis and chemotaxis. In response to debridement, diabetic foot ulcers are stimulated to begin the healing process. In this article, various methods of debridement
are reviewed, along with the potential effects they have on reduction of bioburdens; and enhancement of wound closure is also examined.

Two new algorithms can accurately calculate the absolute risk during a 10-year period for developing blindness or needing limb amputation, two potentially serious complications for men and women living with type 1 or type 2 diabetes, according to research in The BMJ.
In a prospective cohort study of database information collected for more than 450,000 patients with diabetes, Julia Hippisley-Cox, Ph.D., and Carol Coupland, BSc, MSc, Ph.D., both professors at the University of Nottingham, U.K., said the tools — similar to the QRISK2 algorithm to predict cardiovascular risk — are based on variables that patients are either likely to know or are recorded routinely in general practice visits.

Plantar heel pain represents the most common musculoskeletal condition we treat in podiatric practice. Podiatric physicians are not the only caregivers for this problem as over 1 million people with plantar heel pain visit primary care providers and orthopedic surgeons each year. The direct cost of treating this disorder ranges from an estimated $192 million to $376 million annually. More importantly, researchers have documented the effects of plantar heel pain on quality of life and productivity, which justifies the need for prompt and effective treatment.

The aim of this review was to identify and evaluate existing research to determine the clinical effectiveness and safety of prolotherapy injections for treatment of lower limb tendinopathy and fasciopathy.
Two hundred and three studies were identified, eight of which met the inclusion criteria. These were then grouped according to tendinopathy or fasciopathy being treated with prolotherapy injections: Achilles tendinopathy, plantar fasciopathy and Osgood-Schlatter disease. The methodological quality of the eight included studies was generally poor, particularly in regards to allocation concealment, intention to treat analysis and blinding procedures.

Kinematic and kinetic alterations in the lower extremities that researchers have observed during the course of a prolonged run may provide clinically relevant insights into patellofemoral pain and other conditions associated with a gradual onset of symptoms during exercise.
Many people choose running as a convenient and inexpensive type of physical activity. Running has increased in popularity since the 1970s, and not surprisingly the number of people injured while running has also increased. The percentage of runners who experience running-related injuries can be as high as 79.3 percent for lower extremity injuries.

It's not uncommon for doctors to enter the field with mounds of medical school debt and little savings — after all, they spend extended time in school before starting their practices. That can put young doctors at a disadvantage compared to their peers in other professions who have been able to start saving up to 10 years earlier, according to a new study by AMA Insurance, a subsidiary of the American Medical Association. The report looked at 1,937 U.S. physicians under age 40.
